Most ladder injuries do not happen because the ladder was defective. They happen because the ladder was set up wrong. An extension ladder pitched too shallow can slide out at the base. One pitched too steep can tip backward as a worker leans away from the rungs. The single most effective way to avoid both failure modes is to set the correct climbing angle, and the simplest tool for getting that angle right is the 4-to-1 rule.

What the 4-to-1 Rule Is
The 4-to-1 rule is a simple ratio for positioning a non-self-supporting ladder, such as a single or extension ladder that leans against a wall or other support.
For every 4 feet of working length, the base of the ladder should sit about 1 foot away from the wall.
Working length here means the vertical distance from the ground up to the point where the ladder touches the upper support, measured along the height the ladder is actually spanning. Setting the base out at one-quarter of that height produces a climbing angle of roughly 75.5 degrees from the horizontal. That angle is the sweet spot: steep enough that the ladder does not slide out under load, but not so steep that it becomes unstable or hard to climb.
The ratio is easy to remember and easy to apply on a job site, which is exactly why it has become the standard rule of thumb. You do not need a protractor or an inclinometer to get close to the correct setup, though tools and built-in angle indicators are a useful confirmation.
The OSHA Basis for the Rule
The 4-to-1 rule is written directly into the OSHA construction standards. Under 29 CFR 1926.1053(b)(5)(i), non-self-supporting ladders used in construction must be set up so that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder. That is the 4-to-1 ratio stated in regulatory language: a working length of four units corresponds to a base offset of one unit.
This requirement applies to portable, non-self-supporting ladders, the leaning type. It does not apply to self-supporting ladders such as stepladders, which are designed to stand on their own four feet and have their own setup and use requirements.
For general industry workplaces, OSHA addresses ladders under 29 CFR 1910.23. The general industry standard requires non-self-supporting portable ladders to be used at an angle where the horizontal distance from the top support to the foot is approximately one-quarter of the working length, mirroring the construction rule. Whether you are working under the construction standard or the general industry standard, the practical target is the same: set the base out at about one-quarter of the working height.
A note on accuracy: both standards use the word "approximately." The 4-to-1 rule is the intended setup, not a tolerance you must hit to the inch. The goal is to land close to the 75.5-degree angle. Always defer to the ladder manufacturer's instructions and labeling, which may specify the rated angle and any conditions of use.
Base-Distance Table by Height
To apply the rule, divide the working length by four. The result is how far the base should sit from the wall. The table below shows common setups.

For example, if the ladder contacts the edge of a roof 16 feet above the ground, the base should be set roughly 4 feet out from the wall. If it reaches a support point 24 feet up, the base belongs about 6 feet out.
Remember that working length is measured to the point of support, not to the very top of the ladder. The extension that rises above the landing (covered below) is not part of the height you divide by four.

How to Check the Angle in the Field
You will not always have a tape measure handy, and you cannot always measure the base offset precisely on uneven ground. There is a fast, tool-free body check that firefighters and many trades use to confirm the angle is close to correct. It is sometimes called the firefighter method or the elbow method.
- Stand with the toes of your boots touching the feet of the ladder where they meet the ground.
- Stand up straight and extend your arms straight out in front of you at shoulder height, without leaning forward or back.
- Your palms should just reach and rest comfortably on a rung at about shoulder level.
If you have to lean forward to touch a rung, the ladder is set too shallow (base too far out), and it is at risk of sliding out at the bottom. If your hands fall short of the rung and the ladder is right against your chest, it is set too steep (base too close to the wall), and it is at risk of tipping backward. When your straight arms just meet the rung, the angle is close to the target 75.5 degrees.
This check is a confirmation, not a replacement for the 4-to-1 measurement. Many extension ladders also have a printed angle indicator or label on the side rail; line it up if your ladder has one.
The 3-Foot Extension and Securing the Top
Getting the angle right is only part of a safe setup. When a ladder is used to access an upper landing surface, such as a roof, platform, or floor level, the side rails must extend at least 3 feet above the landing, per 29 CFR 1926.1053(b)(1). That extension gives a worker something solid to hold while stepping on and off the ladder, which is one of the most dangerous moments in any climb.
If the ladder cannot be extended a full 3 feet above the landing because of its length, it must be secured at the top to a rigid support, and a grasping device such as a grab rail must be provided to help the worker get on and off. Even when the 3-foot extension is achieved, securing the top is good practice: tie off, clamp, or otherwise stabilize the upper section so the ladder cannot shift, slide sideways, or be knocked loose while someone is on it. Keeping the area at the base clear and, where appropriate, having a coworker hold the ladder during access adds another layer of protection.
Common Setup Mistakes
Even experienced workers fall into predictable traps when placing a leaning ladder. Watch for these:
- Too shallow (base too far out). The most dangerous error. A flat angle increases the outward force at the feet, and the base can kick out, dropping the ladder and the worker. This is the failure the 4-to-1 rule is most designed to prevent.
- Too steep (base too close). A near-vertical ladder is unstable. As the worker leans back to reach or climb over the top, the ladder can tip backward away from the wall.
- Setting up on the wrong surface. The feet need firm, slip-resistant footing. Smooth concrete, wet surfaces, ice, loose gravel, and soft soil all let the base slide. Use the ladder's slip-resistant feet correctly and never place a ladder on boxes, barrels, or other unstable objects to gain height.
- Not level or not plumb. The feet must sit on level ground so both rails carry equal load, and the ladder should lean against a firm, even support at the top. Do not shim one foot with scrap material; correct the ground or reposition the ladder. Never lean a ladder against an unstable top support such as a window, a loose downspout, or a flexible surface.
- Ignoring the top support and extension. Leaning against a corner, a pipe, or anything that can roll or flex removes the stability the angle is supposed to provide, and skipping the 3-foot extension makes the transfer at the top hazardous.
A Caveat on Scope
The 4-to-1 rule and the practices above are general guidance to help you set up a leaning ladder safely. They are not a substitute for the ladder manufacturer's instructions, the judgment of a competent person, or a site-specific hazard evaluation. Manufacturers may specify particular angles, load ratings, surface conditions, and limitations for their products, and those instructions take precedence. Conditions such as wind, electrical hazards near power lines, uneven terrain, and the task being performed may all require additional controls beyond simply setting the correct angle. When in doubt, consult your competent person and the applicable OSHA standard before climbing.
