Documented case: a fatal finishing task
On December 8, 2023, machinist Nick Hardiman was finishing a rotating component with hand-held emery cloth at Somers Forge Limited in Halesowen, England. He became entangled in the lathe and died that evening. HSE found failures to prohibit that method, prevent access to dangerous moving parts, assess the task and establish a safe procedure. It also found the company had failed to ensure workers' PPE did not create a risk of injury from, or being drawn into, the lathe. HSE case report

These are proposed review prompts, not corrections documented at Somers Forge. OSHA prioritizes elimination/substitution and engineering solutions before work practices, administrative controls and PPE; selected controls must not introduce new hazards. OSHA control framework

- Engineering: If the risk assessment selects emery cloth on a manual lathe, review HSE's tool-post holding methods. Adapt together: Where, if anywhere, is hand-held abrasive finishing proposed on rotating stock? What change would remove that exposure? What remains unresolved before the task can proceed?

- Close the loop: Who will report back, by what time, and how will the crew learn whether each unresolved issue has been addressed?
