There is no documented FACE case behind this account.
Picture a regional sales manager at a mid-size firm — the kind of role where "desk work" and "windshield time" blur together. In the week before the holiday, the schedule stacks up: a five-hour drive to a client site, a full day hunched over quotes and a laptop, then a cross-country flight home with a tight connection. Across three days the person sits for stretches of four to six hours with almost no walking. Legs stay bent at the knee and hip, calf muscles idle.
Here is the mechanism that matters. When you walk, your calf muscles squeeze the deep veins in your legs and pump blood back toward the heart — the "calf muscle pump." When you sit motionless for hours, that pump stops. Blood pools and slows in the deep leg veins. Slow, stagnant blood — venous stasis — is one of the classic conditions that lets a clot form, a deep vein thrombosis (DVT). The warning cues are quiet: a heavy, aching, or swollen calf, often in just one leg, sometimes warm or red. It is easy to write off as "a long day."
The real danger comes next. A piece of that clot can break loose, travel through the veins to the lungs, and block blood flow there — a pulmonary embolism (PE). In this scenario the manager felt short of breath with chest pain climbing stairs the morning after travel, was taken to the emergency department, and was treated for a pulmonary embolism. The outcome was a survivable medical emergency requiring hospitalization and anticoagulant treatment.
Timeline
- Three days before — Manager begins a travel-heavy stretch: long drive, full desk day, return flight, minimal walking breaks.
- Earlier in the trip — Assumes fatigue and a "tight" calf are just from the drive; no one flags prolonged sitting as a health hazard.
- Moment before — Notices one calf is swollen, warm, and tender; treats it as a strain and keeps moving through the schedule.
- Point of failure — No movement plan, no hydration routine, and no awareness cue triggered a break in the sitting; venous stasis went unaddressed.
- Response — Sudden shortness of breath and chest pain the next morning; EMS transport, ED diagnosis of pulmonary embolism, hospitalization and blood-thinning treatment; survived.
What Went Wrong (Root Causes)
Hazard
- Hours of unbroken sitting stopped the calf muscle pump, letting blood pool and slow in the deep leg veins (venous stasis) — the setup for a clot.
Procedure
- No written expectation to break up prolonged sitting during long desk days or travel legs; no hydration or movement guidance in the travel policy.
Supervision
- The schedule packed back-to-back sitting blocks — long drive, desk day, flight — with no built-in time or prompt to walk, treating "seated equals safe."
Training
- Staff were never taught that prolonged sitting is a recognized circulatory hazard, nor the early warning signs of DVT (one-sided calf swelling, warmth, ache) versus a simple muscle strain.
What Would Have Stopped It
The highest-order controls here are administrative and behavioral, because the hazard is the design of the workday, not a machine. The hierarchy of controls runs elimination → substitution → engineering → administrative → PPE — and the honest question for the crew is: what is the highest-order control we can use TODAY? Eliminating uninterrupted sitting is the goal: build movement into the schedule so no one sits for hours straight (administrative). A standing rule to stand and walk two to three minutes at least every hour, in-seat calf pumps and ankle circles on long drives and flights, and steady hydration (dehydration thickens blood and worsens stasis, a real summer and travel concern) are all administrative controls. Sit-stand desks and walking-meeting norms are engineering-type changes that physically reduce forced sitting and support the same goal.
None of this replaces medical judgment. Anyone with one-sided calf swelling, warmth, and ache should treat it as a possible DVT and seek care, and sudden shortness of breath or chest pain after prolonged sitting is a call-911 emergency. Under OSHA's General Duty Clause, Section 5(a)(1), employers must furnish a workplace free from recognized hazards likely to cause serious harm — a frame that reaches sedentary and ergonomic exposures, not only falling objects.
